30PM Duties and responsibilities include, but are not limited to, the following:
Reports to and assists the DON, responsible for managing and directing the daily nursing and care in assigned unit and backs-up the Unit Coordinator/Care Coordinator team.
Assesses the quality of care rendered and works with nursing leadership in performance improvement.
Works with the overall community to assist in defining, implementing and evaluating the care and services in a person-centered environment.
Within the Unit, assists the DON to investigate all resident care issues and coordinate action plans.
Collaborates with other staff regarding resources and services for improved resident care and staff development.
Directs the delivery of quality resident care.
Responsibility includes but is not limited to:
management of antipsychotic/psychotropic medications, assisting with care plans, coding, management of documentation, incident report tracking with follow-up, infection control and antibiotic stewardship, laboratory tracking, QAPI leadership, quality measures, restraints, self-reporting incidents, survey management, assisting with staffing, and audits to ensure consistent high quality of care and service.
Consistently supervises nursing staff in the Unit.
Assists the DON to ensure completion of assignments by mentoring, monitoring and tracking progress.
Meets with physicians, families, and agencies representing St.
Joseph Senior Living.
Completes other assignments as deemed appropriate by the DON.
Assumes leadership responsibilities.
